choosing an insurance plan!!

Hi it is open season for government employees, and I can choose a new insurance plan, yea!!! This is what the plan I am considering on choosing, do any of you think it will work, so I can get my surgery? It says, Surgical treatment of morbid obesity a diagnosed condition in which the body mass index is 40 or greater or 35 or greater with comorbidities, such as diabetes, coronary artery disease, hypertension, pulmonary hypertension, weight related degenerative joint disease or lower extremity venous or lymphatic obstruction. Eligible members must be 18. OK I have depression, and degenerative dis disease, and chronic hip pain. I called the insurance company and spoke to one of the nurses, and she said they also want the pcp records, for the depression and degenerative disc disease. Along with a psychologist, pcp letter, nutritionist, and surgeon letter. I have all this cause I was prepared to have the surgery with my husbands policy, but found out it is selffunded, and must have a bmi of 40 or be 100 pounds overweight, so I don't qualify for his. Is there anything I need to ask, or find out, any information is greatly appreciated. My BMI is 35.5. So the nurse said I qualify, but this new policy does not take effect till Jan 12, 05. thanks
