Denial from Insurance company

Here is the deal:Midwest Security-PPOM They denied me because and I quote " Coverage for non-experimental, surgical treatment by a health care provider for morbid obesity that has persisted for at least 5 years and for which non-surgical morbid obesity treatment that is supervised by a Physician has been unsuccessful for at least 18 months." I need help to fight this. I can request a review by sending a written request. Has anyone had this problem and if so, what did they do to fight it to win?
