Need help with a SECOND letter of appeal for company self-insured

with UHC California. They have a written exclusion for obesity surgery, but it does not state anything about medically necessity. I sent the first "kick-ass" appeal letter, and I know they are going to kick it back down, I am hoping for an outside review. Any suggestions?
