Some of you have read some of my posts, so you know that I have been denied coverage by an insurance company that knows my employer will be switching coverage. A friend of mine also tried to get weight loss surgery and was told she met all of the requirments EXCEPT she needed to see a dietition 3 more times. This was something they never mentioned to me!! They are just adding conditions to getting the surgery so they don't have to pay out for it. And if I say anymore about insurance companies at this point, my language will become quite inappropriate.

Go by the requirements listed in your first denial letter. They cannot continue to add requirements - you really need to fight this. This is considered "Unfair Claims Practice" and not legal. Fight the good fight! Roberta
