nyc employee / ghi empire bc bs

Does anyone know exactly what I need to send them to get approval on the first shot? Getting nervous!!!!!!!!!!

Fellow New Yorker here. Don't worry. If you don't you press on and you can appeal to NYS which has been very great. First shots are hard, but the State is firm. I never undersgtand why they make us appeal to the State knowing we win but I think they exptect people to walk away. Good luck Gary www.obesitylawyers.com
