looking fro advice on how to proceed plastic surgery

i am about to start looking for a plastic surgeon to get tt and breast surgery. however i was not 100lbs overweight to begin with ...so not sure what to expect ..and i want to get an early start on researching i need to know know if anyone has copies of appeal letters i have bcbs of ga hmo ...and what i may need to submit for initial review any help will be appreciated.
