Insurance denied...need help to appeal, please!

Is anyone willing to share a sample appeal letter? I just found out yesterday that my insurance company (CareFirst/BlueChoice) denied my WLS because of a written exclusion for small businesses in Maryland. I refuse to give up without a fight and I intend to go to MIA (Maryland Insurance Administration) if need be. I NEED this surgery soon! My health is going downhill fast! Thanks for reading this post.
