Anthem Blue Access?

It appears this insurance is out of OH. This is the insurance that will be available to my husband shortly. Does anyone have any information on how they approve for surgery? Would the DS be included? Any information about this insurance is greatly appreciated. Thanks in advance, Elizabeth preop
