Insurance denial :(

I was recently denied by Healthlink because they said i haven't proved medical necessity and i need to do a 6 month medically supervised diet and behavior modification and exercise program. I thought I provided enough diet and exercise information. I need to write an appeal. Is there anyone who can help me? And if i write an appeal, what are my chances of approval?
