Humana PPO

I am seeking a revision from a VBG to an RNY. I have Humana PPO. I throw up at least 3 times a day. The upper GI revealed no staples, no band. In the EGD, the doctor said he saw the band and staples.....the pouch is stretched. Has anyone used Humana for a revision, and what was the criteria? Is throwing up, lower back pain, significant weight regain and knee pain going to be considered medical necessity? Thanks for any help..
