5 years documentation???

Has anyone else been asked for 5 years documented weight loss attempts?? I have Herrington Benefits insurance. Also, my primary care physician has offered to write the appeal letter for me, but should I also write one on my own? One more thing, my plan states that bariatric surgery is covered with 5 years of documented medical records OR if it determined to be medically necessary. Now they are saying that to prove it medically necessary, I need to provide 5 years of documentation. Is there any hope?

is that 5 years of Morbid Obese history? or 5 years of Diet attempts? I know I have Aetna and I have to show proof of 5 years of 100lbs or more morbid obesity. which I am showing 2000,2001,2002,2003-2004 weights of 100lbs and finishing my dr supervised diet in Jan. I had also for my dr wrote down in nice format of my years of diet attempts of what I've done /tryed and how much I have lost..from my beginning years to present. of course I had to do the dr diet because of having to be within 2 years prior to surgery. The answer from Shauna is basically what I would say. Medical Mutual of Ohio requires a 5 year history....of weight.....obviously over 100 lbs. With a letter (I also asked and got from my PCP a letter recommending I have surgery) I was approved within 10 days. Every carrier is different...so call...get specific information, and act accordingly..... Pete [email protected]
