denial x's 2

the reason for denial is the want 6 months of DR monitored weightloss plan. I submitted weights from my pcp and other mds but they wanted 6 consecutive months. I found this not to be fair as I know several other people with the same ins GHI/BC/BS PPO who did not have to submitt this . one individual was approved 1 month before me and has since had their surgery.
