Dr letter for insurace

Does any one have a sample letter I can use for the insurance from my primary. He said to just wright a letter and bring it to him to sign. I am trying to get approval before the new year when my insurance will change. Please help Amy

Hi Amy..I just used the one from here..and filled in all the important stuff about me and added some things and whala! my pcp was pretty impressed just make sure that it states in there that "MEDICAL NECESSARY"(sp?)and MORBID OBESE...is a must..not over weight..not obese..but MORBID..If I had mine on hand I would share...Good Luck!
