Employer is Changing Insurance Carrier

I am not sure what mu options are. My employer had John Deere insurance and I was approved for an evaluation by my bariatric surgeon in July. I have been waiting for my appointment and now have one for Nov. 21. The problem is that my work is changing carriers and the new one has a bariatric surgery clause- I have to be covered for one year then they will cover only 60%. I have spoken to the surgeon's office but they can't say if I have any chance of having my surgery before the end of the year. It would be doubtful to be able to get through all the pro-op stuff and scheduled by then. Any suggestions as to what I can do? I am angry and disappointed.
