Appeal Letter for Tummy Tuck

I am 7 months out and 115 pounds down and have a large insicional hernia that needs to be repaired. My Dr. submitted to the insurance for the hernia repair and a panniculectomy (sp) for the large apron of skin I have hanging down...they denied the panniculectomy and will only pay for the hernia. My Dr. agrees that it really is the best time to repair everything even though I am still 75 pounds from my goal. Has anyone appealed to thier insurance company and won and do they have a copy of the letter that I could look at to get some ideas?
