What does a 6 month supervised diet require?

Does any one know what documentation is required for the new BC/BS guideline for the 6 month supervised by a physician diet?

Janet, I just completed mine last month and was finally approved after 2 denials, (my surgery is this coming tuesday). I went to my dr once a month and he documented my weight and blood pressure. At the end of 6 months he wrote bc/bs a letter including my weights and b/p and also included the type of diet I was using---such as a 1600 calorie diet and he also included that I was walking and exercising twice a week. He told of my numerous attempts at weight loss and failures and my extensive family medical history as well as my high blood pressure. Also, the more documentation you can give insurance company the better chance you have. I wish you luck.
