does anyone have bcbs MI community blue ppo-

My surgeon is getting together my stuff to send into the insurance co...bcbs of MI community blue ppo, and hearing all these horror stories of not being approved. They will approve, but I heard you need 3 years of documented weight loss. I have 18 months of weigh****chers, but other than that, I've been on everything from A to Z, but just have no proof...alot of which I did on my own. Anyone recently request approval from this particular company? I understand it's been just real recently that they are requiring the 3 year documentation. Are they very strick on three years, or can I slide by with what proof I have? Any comments welcome! Thanks! Theresa
