2nd insurance denial

Please help! I have Cigna of NC Open access. I weigh 245 with a height of 5'4and a half with a BMI of 41.I am planning to have a revision.I had my 1st surgery in November 2001. I continue to have high blood pressure and gout.I can't boost up on protein r/t kidneys.I just heard thru my inurance company I was denied a 2nd time.Has anyone had Cigna for a revision.The customer service person stated I may ahve been denied because I had already had the surgery.Any suggestions would be greatly appreciated.
