Insurance letters on this website / BMI ... estimated % rate of death?

I am preparing my letter to the insurance company and can't find how to figure out the estimated percentage of increased risk of death at my present weight. This is on of the "fill in the blank" parts of the letter but I can't find anything about how to figure it out anywhere. Can anyone help me with this? Thank you, Tamara Carman
