Writing letter to insurance (history)

I know that I saw a really good letter somewhere on this site (not sure if it was in the insurance part or not) that the writer said others were welcome to use to help them in writing their own letters with their own symptons. Does anyone know what I'm talking about and if so, can you help point me to where I might find it. Thank you.
