Great PCP letter needed

I need a sample of a great PCP letter to my insurance requesting GBP surgery for medical necessity.

did you get a letter yet or do you still need one? I have a few I've been collecting from different people, primarily they are post-ops, explaining how life has improved for them after surgery. The letters came from people who's insurance did cover the surgery. Is there anything in particular you need the letter to say? I could probably check with some of those people that wrote some really good letters to see if they mind if I forward one or two to ya. Let me know what you're looking for and I'll see what I have and then make sure people are ok with me passing it forward, I'm sure it'll be fine though. They are all doing it to help someone who has been thru all the appeals and was still denied. So again, let me know what you think you're looking for and I'll see what I can do.
