Denied...please help steer me on how to write an appeal

I just recently found the site somewhere in this stuff that had some people post examples of how to write a great letter for an appeal. If anyone could help me find that site and tell me how to write the letter I would be forever grateful. I need to lose just over 100 pounds, but the insurance said my BMI wasn't high enough to put me into morbid obese catagory. Gee, It's 38 and climbing. Haven't been able to do a lot of exercise due to chronic pain in several parts of the body. anyway, sorry for the long post and thank you in advance.
