Letter To Insurance

I have one more step to do in order to be able to file for my insurance. My doctor has requested that I write a letter to the insurance company with a list of my diet attempt and failures. Throughout my research of WLS I found an example of a great letter to the insurance company SOMEWHERE on this computer. However now that I need it I can not find it anywhere. Does anyone know where I can find an example letter? Thanks for your help

This is great - I have my letter from my PCP and am getting everything together to send to the surgeon's office - I have Cigna and I am going to use this letter for sure. Thanks - Thanks - Thanks!!!
