If you are appealing an insurance decision, this url has some extremely helpful information. These are the "Clinical Guidelines on the Identification, Evaluation and Treatment of Overweight and Obesity in Adults." In other words what the insurance companies use to determine their criteria...its often called the NIH/NHLBI 1998 Consensus Report guidelines......
