Need example of letter from PCP

Hi, I plan to talk to my PCP on August 30th about getting the surgery again. The first time that I talked to her she was against me getting the surgery. She said that I was not heavy enough. So she asked that I follow a diet plan for 3 months and if I didn't lose the weight she would support me. I need an example of the letter she should write for me recommending that I have this surgery. All responses will be appreciated.

Hi Hope! I found my pcp letter here, theres a few for insurance purpose and appeal also, I just rewrote it to fit me and then gave it to my pcp and she just rewrote it over again with her words on her heading paper and then signed it..it actually was a template and worked out great, also if you type in letter in the search then you might come up with different types. Also it helps I think to get other dr's you see, to write one also for what co-morb you have..like I did with my podiatrist, I have bad feet problems, and my gyn dr wrote a wonderful letter for me for my PCOD. anyways you get the idea, good luck with your journey and as for me, i'm having to do the 6mo diet thingy again to get approved!
