Anastesia not covered!!

Hello all, I got approved for surgery, but when the anastegiologist sent their bill the insurance denied it?? said it was out of network...how could the approve the surgery, but not the anestegia??? was I supposed to go under the knife awake?? anyone else have this problem??? how did you resolve it??
