Humana ChoiceCare PPO

Hello, I need some information please. I found out today that when my employer set up the insurance benefits they excluded coverage of weight loss surgery. Has anyone else had any dealings like this and been able to appeal it and get approved? I would really appreciate any input at all. Thanks to everyone! Pat
