Plastic surgery

I am having trouble with Bluegrass Family Health (Lexington, KY) on getting approval for the removal of excess skin after I have lost 155 lbs. Does anyone have any suggestions on how to go about an appeal? I just have a couple of days to send them a letter and am very depressed about it. Thanks so much, Kathy Anderson
