Sample Letter of Medical Necessity from PCP or Surgeon

My PCP has admitted that he has very little knowledge of WLS. I was very disappointed to see that the Sample Letter of Medical Necessity from PCP was not available from this site. The other sample letters are outstanding!!! Is there anyone out there who can send me a copy of what their Dr. submitted? (Minus any personal information you don't care to share, of course.) I want to have the letter ready for my doctor to either just sign, or for him to use as a guide.
