Revision Appeal Letter

I had a VBG in '92. I am looking for help/suggestions in writing a revision appeal letter. With having the VBG a dr. supervised diet is pretty much out of the question. Do I keep the letter short and to one page like a resume or do I get detailed with charts and tables. My insurance is CIGNA. Any and all help is appreciated. Thanks, Piper
