Health Net/Arizona

Has anyone had any success in getting approved by Health Net in the state of Arizona? I have been denied and now I am ready to start the battle, but I wanted to first start by seeing if anyone out there has been successfull. I will be having the surgery with Dr. Hilario Juarez - if anyone has any info on how his office works with Health Net that would also be greatly appreciated. Thank you.
