denied ins coverage for surgery

I have First health Ins thru Boilermakers Health and Welfare Fund I had intestinal bypass surgery 25 yrs ago, did not have enough weight lossbut still have negative side effects from this surgery Insurance co first said they would pay for reversal surgery but not gastric bypass surgery. Now they are refusing coverage on both. Has anyone had a similar problem? Has anyone had surgery covered by First Health? Please reply.
