Minnesota Health Insurance Help!

I feel like I am losing my mind trying to find an insurance plan to switch through for WLS in Minnesota. I was on Tricare Prime because my husband was an active duty marine. He was medically separated, and now we have lost our coverage through Tricare. Please help I have been looking up plan after plan and have no been able to find one that covers bariatric surgery.
