Any luck with UHC in Maryland

I am starting the process to get approval for VSG. I have UHC and was told it is covered by my plan but they have a third party that handles everything. They are asking about a 6 month weight lose plan and other forms to be filled out by my doctor. I would like to use St Agnes Hospital and Dr Averbach. Has anyone had any problems getting the approval from UHC. My BMI is 43.5 plus I have hyper tension and pre diabetic. I was told Maryland is actually one of the few states that require it to be covered by insurance. But I also heard UHC is very difficult to get approval from.

If you meet the plan's criteria, you will get approved. IMO, UHC did little to try to connect any dots, so make sure your submission is water tight and you should be good to go!

I recommend you get a copy of your plan's guidelines if you haven't already. A call to customer support can tell you if your provider/hospital are in network.
