SWA-United Healthcare Choice Plus

Has anybody here been able to get UnitedHealthCare to cover WLS working for SWA?  I'm in the beginning stages of research and want to know what type of challenges I may face along the way.

I'm on UHC through ING, and my case went before the ING appeals board today. From my research, it seems that if folks meet the criteria they're approved, but if they don't, they're denied. My plan requires a BMI of 40 and 5 years of documented comorbidities. I was at 38.8 and was denied multiple times. I hired Walter Lindstrom's office to help with the appeal and it's a good thing. We've had some hurdles to clear with this last appeal and it's been nice to have someone else handle it.
