Network insufficiency?

Hi everyone!  I just found out that the one Indiana facility that my health insurance will cover does not perform the duodenal switch.  Being 50+ BMI, among other factors, that's the surgery I want. 

Do you think I have a case to go to my insurance company and see if they will cover another instate facility that does perform the DS?  I am currently looking into having the surgery in an adjacent state, at a facility that the insurance will cover.  But I am finding coordinating the preop requirements an uphill endeavor with a surgeon and facility 200 miles away.  (How do the Mexico people manage it?)
