Going for the duodenal switch surgery later this year or early next year.  So, I contacted a surgeon about the procedure.  So, okay, I got the >50 BMI, check.  I got the federal BC/BS insurance, check.  I dig the head shrink and the food shrink evals, cool.  But having to document a 3-5 yr history of weight loss attempts with receipts is ridiculous.  What's even more annoying is that I've called my insurance and nobody said I had to have paper receipts of this nightmare!  I'm ready to chuck the job, the insurance, grab myself a shopping basket and call it a day.

Poop on a stick!  Can somebody throw me a hope rope here?

Not sure if this will help because my insurance only requires a five year history of obesity not weight loss attempts, but I hadn't been to Dr sincy my last baby was born six years ago before I started this journey.  I am hoping to get by with my prepregnancy weights from my last three babies, starting in 1997 (all obese) and pics of the last five years (with dates on them) showing my morbid obesity.  Haven't even submitted yet so it probably isn't much help.

Weigh****chers, etc, should have some records if you went to someplace like that.  A gym should be able to give you a print out of times you went, etc.  

Believe me, I feel for you.  If I had the money I would self pay and forget about the insurance hassles.

Do you possibly have canceled checks or credit card statements showing where you paid for these items? I know I attended Weigh****chers for several months and had the monthly pass charged to my credit card, ordered Medi-Fast products on my credit card,  and ordered Meridia via my prescription plans direct order which has limited records available online (showing the medicine and refill dates) that could be used along with credit card statements showing payment. Most credit card companies have statements available online and you can request older statements to be made available. I think mine have 3 years worth available upon request (can only request a couple at a time), but the previous six months are always available.
Did you keep signed copies of any contracts you signed for gym memberships where they obligate you to so many months at at certain price? I would think that would suffice, too. You may have to produce bank statements showing the deduction from you account each month to the gym. If you don't have the copies of your contracts, you should request a copy.
Sounds like a huge undertaking, but can be done.
