Keystone Health Plan East Questions

 So I have KHPE and have a few questions.  I started this process back in Feb. but gave up because I had an issue with the psych eval and now have chosen a different surgeon and program to go through.  I originally was going to go through Pennsylvania Hospital and they told me I had to do a 3 month supervised diet.  When I called my insurance, they said it was not required??  Right off the bat, the surgeon's staff was like, who's your insurance?  I told them, and got "oh you have a 3 month supervised diet..."  I told them I had called my insurance and was told it was not required.  They just said that the insurance company required it and that was that!  I really thought they were just out to get money....Anyway, I am now going to go through Einstein (a good friend had her surgery there and loved them! - she also has KHPE and didn't have to do a supervised diet).  

Anyone else with Keystone Health Plan East have to do a supervised diet?  Also, was your psych eval covered (not including copays..)?  Oh and one more, how was the approval process?
