Kentucky Medicaid?

Hello Everyone, I am new to this website and am VERY interested in having a gastric by-pass.
I am from N. Kentucky and have medicaid. Can any one help me with information on how to go about getting Medicaid to cover it. I am 250 lbs and am only 5'2" I have high blood pressure, diabetes, a large hernia, and cronic lower back pain. I am really in need of advice please help.
