Approved Today! Guardian PPO
Guardian Required The Following:
Letter of medical necessity: Provided by the surgeon.
Nutrition Consultation: Provided by the PA at the surgeon's office.
Medical Clearance by GP: Helps to see a doctor who supports WLS. I was given a physical and a heart scan.
100lbs overweight and/or double your ideal body weight: I did the work on that one :)
Psych Evaluation: Was referred to a very nice therapist, who then wrote a letter.
6 months supervised weight loss: I had records going back from 2002, from my time with a bariatric physician. I had 2 different weight loss attempts, totaling about 2 years.
Once I had all the doctor's visits the insurance required, the insurance coordinator at the surgeon's office faxed it on Wednesday. I was approved the following Monday. Guardian seems to be very good.
Insurance approval was a big worry of mine too, so I would be happy to answer any questions!
