Blue Advantage HMO -- Illinois

My employer changed insurance policies and I need to find a new primary doctor in the NW suburbs who will give a referral for bariatric surgery.

This is very frustrating because I was on a PPO and was in my third month of the supervised diet and getting ready to schedule the surgery.

If anyone has BCBS Blue Advantage and has a suggestion I would appreciate your help.
