Amy,
First thing you have to do is a 6 month supervised diet. You have to go to your doctor discuss weight loss strategy and have monthly weigh ins. You cant miss a single visit and the  dr MUST chart  you strategy and progress.  once you hit about 4 months you should go to a seminar( i recomend  Premiere bariatrics out of dayton)They will give you a list of things to have done ... blood work  for hpylori  ect.. you MUST have clearance from a psychologist and do 2  personality tests as well as an interview. You must get  clearance from a cardiologist and a pulmonogist.YOu have to have documented co morbidities  like high BP sleep apnea arthritis and so on.... I have a list  and i can send you more specific requirements tomorrow. I dont have it in front of me right now.. good luck for now

Hi Amy,

Congradulations on your date. I am currently starting the process with Molina and like you have been morbidly obese for over five years. I have MS, lower back problems, arthritis, and sleep apnea and was glad to see that they didnt have you do the six months of dieting which leads to my question as I am hoping that I dont have to as well. What makes Molina decide to have some do the six months of doctor supervised dieting and not others?

Thanks
