The dr that originally did the surgery does not practise in NJ anymore

I have Horizon NJ Health

The best way to do this is to call your ins co and get a list of all the bariatric docs they contract with and narrow the list from there,

Depending on which revision you are looking for the list of potential docs could be long or short.  For example, if you are wanting RNY to DS there are very few doctors that even do that procedure but if you want band to RNY the list will be quite long.
