Any info on BCBS of AZ?

I am switching jobs and insurance. So, the first thing I did was investigate the benefits. As far as the employer is concerned, bariatric surgery is covered. Here is the language outlining the EXCLUSIONS:

Your medical program will not provide benefits for services, supplies or charges:

• For weight reduction programs, including all diagnostic testing related to weight reduction programs, unless medically necessary and appropriate.
• For treatment of obesity, except for medical and surgical treatment of morbid obesity or as provided herein.


However, when I went to BCBS of AZ's website, I could not locate the medical policies outlining the actual criteria you have to meet to satisfy the insurer. Can anyone assist me with locating this information?

It doesn't appear that they post them on their website.  Use the contact us tab, and send a request for a copy of the medical policy concerning bariatric surgery.

I actually sent them a request last night, but I wanted to see if anyone had the criteria (in case they do not return my request) or if anyone had personal experience with this company.
