Personal Appeal letter

I have been denied by BC BS and appealed once. I have gone to the doctor and found that I have additional cocomorbidity's that I did not know I had. (sleep study confirmed apnea, borderline high cholesterol, and high blood pressure). I am going in for my physical and asking my PCP to write a letter including these morbidity's. I also want to write a personal letter, but I do not know what to include. Does anyone have a letter they used that they would be willing to share? I am also going to look on obesitylaw, and obesityaction to see if they have any. I want to make sure I am including all necessary points.

Why were you denied to begin with? Not a high enough BMI? What surgery type are you looking for?

if you put help with appeal letter in search it will bring up some leters you can use as an example, i'm in appeal myself right now because they  say the surgery i want is investigational,  best of luck
