Empire BCBS and VSG?

Does anyone know if Empire Blue Cross Blue Shield covers the VSG for "lightweights" with a BMI of 35-40?

I am assuming you are with BCBSNY/Empire Blue?
If so here is the link to their medical policy on Surgery for Morbid Obesity...It breaks down what they do and dont cover and all the requirements.

It should cover all plans but you may want to just check with the Customer Service people to be sure. I know for BCBSNC medical policy covers all plans.
