CareFirst of MD

Can anyone give me suggestions on how to write my letter/application for Care First of MD? What forms do I need? What supporting information do I need to know about? Any suggestion and/or help would be much apprieciated.

Are you writing an appeal letter or submitting for insurance approval? Insurance approval submission is generally handled by your doctor. If it is an appeal letter, your insurance policy should have instructions on how to appeal.
