Medically Supervised Diet Visit Form That Someone Posted.
Does anyone have it by any chance?
I believe it was posted by a lady who worked somewhere where she dealt with insurance approval, and she uploaded a form that she suggested we take to our doctor each month, to make sure everything we need discussed is discussed, and then signed by our PCP.
