Ann -  I have the same question! When I talked to my doc he said it was not covered by IBX but when I asked the ins co they would only say that it has to be medically necessary.  There are some others with blue cross insurance that have gotten a peer to peer appeal and gotten it approved after a denial but since all blue cross insurances are run separately IBX might not wok the same way

Good luck!

If they do not cover it unless it is medically necessary you will have to come up with a reason why you can't have the procedures they do cover.  Do you need NSAIDs for an on-going problem?  Do you have a medical condition that prevents you from having a procedure with malabsorption?

I had IBX personal choice (PA) They would not cover the sleeve for me. I did not have any exclusion in the policy. I have fibromyalgia and my doctor sent documentation saying due to the need for ibuprophin, yada...they said no, peer to peer they said no too.
Maybe this year since they are getting the codes for the sleeve it will be different?
Best of luck to you!!
