It varies - sometimes it is quick, sometimes it can take a few weeks. A lot depends on the carrier and (perhaps) the type of coverage you have.

If you have employer based coverage (and your employer is not a governmental unit), then ERISA applies (see link above). If your employer coverage is fully insured then any state mandated timeframes (if stricter than ERISA) applies. The key is you must know what state applies - you may live in TX, but if your employer is based on Illinois, then most likely Illinois' laws would apply to your coverage.
